Breathless Episode 4: The final call for the indefinite strike takes up a majority of the storyline for episode 4. The senior doctors including Dr Moa and Leo take the lead to call for a protest for the betterment of the public healthcare system. While all the juniors unite to fight for Rodrigo, May resists. She considers the strike to be against the principles of being a doctor and is outcasted from the bunch. 

In the meantime, Dr Donoso finds Rodrigo’s belongings, including the multiple phones that he used for livestreaming. Frustrated by the thousands of messages on the phone, she tries to find the password to shut it off. Lluís apologizes for being an absent boyfriend to her due to his son’s incarceration. He promises to have dinner with her that night but calls it off at the last moment.

Dr Moa on the other hand has problems of his own. In order to postpone President Patricia’s surgery, he makes excuses of needing to do another chemo. When she declines, he rushes her surgery to the same day, to honour the rules of the strike. But, a suspicious Dr Amaro moves her surgery to the night, knowing Dr Moa won’t be able to leave for the strike then.

A fatal pregnant patient comes in and is taken to emergency services immediately. Efforts are made to keep both mother and child alive, but the strike calls for a transfer of critical patients to the nearby hospital. While May disapproves, Rucío approves the transfer, hoping for a seamless transition.

However, complications arise as the premature baby is made to be taken out of the dying mother right in the halls. While the surgery is a success, Rucío gets into trouble with Dr Amaro for making such a reckless decision. May and Rucío also break up over fundamental differences. 

An apprehensive and scared Patricia talks about life and careers with Dr Moa to distract herself from the surgery. Meanwhile, Dr Donoso makes her way into Biel’s house to retrieve Rodrigo’s passwords. At the same time, Óscar barges into Enrique’s room next door, and both couples romance each other.

However, Enrique fears sexual intercourse due to Óscar’s STD diagnosis. Back at the hospital, Leo and Lluís discuss their son’s trial. Hesitating to tell her ex-husband about the forged report, Leo mistakenly blurts out the news of the strike. Before Lluís takes any sort of action, the press is called and the news is spread. 

Meanwhile, the president goes into surgery before the news of the strike can be broken out to her. As the clock strikes midnight, Dr Moa wishes to terminate the surgery for the strike, but Dr Amaro insists on continuing. The juniors now, must make a choice, and while the others leave to honour the strike, Biel stays on.

While the surgery goes well, Patricia’s breast is removed and the cancer still stays. This happens due to the size of the tumour which could’ve been gauged by an MRI scan that Dr Moa skipped due to scarcity of time. Patricia feels betrayed about losing her breast and being kept in the dark about the strike.
